Instructions for completing and filing a claim
- Download or print the Claims for Damages Form.
- Complete the City of Federal Way Claim for Damages form in full.
- Sign and date the claim form in the presence of a Notary Public. Notary services are available at City Hall for a fee.
- Attach all relevant supporting documents, such as receipts, bills, invoices, estimates, photographs, diagrams, or other evidence. Please note that all submitted documents are subject to Washington State public disclosure laws.
- File the original notarized claim with the City Clerk for it to be considered valid. You may return the original claim form and attachments in person or by mail to:

Note: In accordance with state law, only completed original notarized forms will be accepted. Forms submitted by email, fax, or any other electronic method will not be accepted and will be rejected. For questions regarding submission, please contact the City Clerk Department at (253) 835-2540.
After the claim is filed
Once a claim is filed with the City Clerk, it is forwarded to Risk Management within the City Attorneyβs Office for review and processing. The involved department will provide information used to evaluate the claim, after which the Cityβs insurance will respond in one of the following ways:
- Accept or settle the claim (in whole or in part) as a compromise.
- Deny the claim (when there is no evidence of negligence or liability on the part of the City).
- Tender the claim to another responsible party.
If your claim is accepted, you will be required to sign a release and complete an IRS W-9 form before any payment can be issued.
